Any door prizes will be appreciated (maybe a credit with your company or other item). Prizes will ONLY be awarded to drillers!

On this form you can sign up as a 4-person team or as a single player. If you want to sign up in any other configuration, please sign up individually as single players.

Corporate Sponsors - $700 includes 4 golfers, sign at lunch and registration. Special recognition at awards lunch, Well Show, banner & in newsletter. Includes advertising on the CART GPS during the entire round!

Gold Sponsors - $600 includes 4 golfers and a hole sponsor sign

Format: 4 Man Scramble. Everyone hits their drive and then plays the best shot into hole. Teams will have a team handicap based on players handicap or average score.

We need all registrations by Aug. 20th & hole sponsor info. for signs as early as possible but no later than Aug. 15th.

We can’t promise hole sponsorship signs any later than that.
